Complete a form questionnaire or form
Forms and questionnaires are integrated into the myKindo payment process by your school or organisation. Sometimes your organisation may choose to set one up without any amount to pay.
To fill in a questionnaire or form, select the item as normal.
Once selected, if a questionnaire or form is required, it will display onscreen for your completion. Anything marked with an asterix * is a compulsory question.
Answer the questions and select Done, to add the item to your cart.
Select Checkout and then Complete Order.
IMPORTANT: Even if the item has a $0.00 value, you will still need to complete the checkout process and place the order for the information on the form to be seen by the school.
